2. Before answering the questions in Section C, the preparer of this form should review the exempt
minor action list,policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. An%action-will be evaluated as to its significant
beneficial and adverse effects upon the coastal area(which includes all of Southold Town).

Dear Trustees:
We are in application for an amendment to a permit-granted by you last year, to construct a new
house and associated site improvements.
Since`that application was file, we have received a permit from the NYSDEC(see attached).
NYSDEC objected to the mechanical removal of the bamboo forest, the stone'revetment, the
retaining walls not- required for the sanitary system, and .the "bubbler,' system proposed by
retired Town Engineer, Jamie Richter, and approved by, you. They proposed instead hand
removal of bamboo, 'revetment' by coir logs, and a-conventional galley system for storm water
runoff. These changes will result in less impact on the site and surrounding, and we hope they
will earn your approval.
The sanitary system design, a 'conventional system with leaching galleys located beyond
Trustees jurisdiction, has not changed.
Therefore, please update our file for a AMENDMENT to our current permit, incorporating these
changes. Attached find five (5) sets of revised SITE PLAN and PLANTING,PLAN, a revised
PROJECT DESCRIPTION. A check for$50 was included with original amendment application.

you preparatory to our PUBLIC HEARING in October, and to attending that meeting via Zoom.

PROJECT DESCRIPTION
Install silt fence and hay bales at perimeter of site;
Eradicate existing invasive bamboo from site by the following method:
cut all bamboo to just above grade;
hand excavate all plants and roots within perimeter of property;
Construct new two-story frame residence on masonry foundation, with footprint of 2,598 sf,
over flood compliant crawl space;
including attached one car garage;
Construct covered masonry entry terrace of 138 sf,
and masonry walkway to grade of 226 sf,
Construct new frame deck on waterside of 544 sf;
of which 280 sf is covered;
Provide connection to public water supply;
Provide underground propane tank for fuel;
Construct concrete retaining walls along property lines: 6q,If along private road and 65 If
along adjacent property to east, for containment of sanitary system per SCDHS;
Install new galley conventional sanitary system per SCDHS, with required waterproof
concrete retaining wall of total 174 If,
Provide "wick hole" under leaching pools area, penetrating .clay layer and providing leaching

Samuels &Steelman Architects on behalf of STEVE & MARCIA DONADIC requests a Wetland
Permit to eradicate existing invasive bamboo from site by approved methods consisting of cutting
all bamboo to just above grade, excavate all roots and soil down to 36" at perimeter of property,
construct retaining walls at front and sides of property, within retaining walls excavate all roots and
soil down to 36", full width of site, in strips 20 feet wide (seven strips total), truck excavated material
to approved site, and place clean sand fill in excavated strips, one at a time, compacting as placed;
propose to construct a new two-story frame dwelling on masonry foundation with a footprint of
2,598sq.ft. over a flood compliant crawl space, and including an attached one-car garage; construct
a 138sq.ft. covered 'masonry entry terrace and 226sq.ft. masonry walkway to grade; construct new
544sq.ft. frame deck attached to seaward side of dwelling of which 280sq.ft. is covered; landward of
dwelling install a new galley conventional sanitary system per SCDHS with required 210 linear foot
long waterproof concrete retaining wall; construct a 1,943sq.ft. permeable parking area with
curbing; construct 130 linear foot long retaining wall adjacent to neighboring property to west; place
approximately 1,400 cubic yards of clean fill throughout property; provide topsoil and landscaping;
provide leaching pools for storm water runoff, provide underground connections to public water
supply; provide underground propane tank for fuel; and to establish and perpetually maintain a 20'
wide non-turf buffer behind the landward edge of wetlands line with a 4'wide maximum access path
to beach. Located: 1071 Bay Home Road, Southold. SCTM# 1000-56-5-37
The action has been reviewed to Chapter 268, Waterfront Consistency Review of the Town of
Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy Standards.
Based upon the information provided on the LWRP Consistency Assessment Form submitted to
this department, as well as the records available to me, the proposed action is recommended as
INCONSISTENT with the below listed LWRP policies.
Test hole results indicate that groundwater was encountered 1.8' below the surface. The shallow
depth to groundwater poses challenged in the development of the parcel.

1. The height of the retaining walls could adversely affect the community character of
the,neighborhood. Screening of the walls is not proposed.
Policy 4. Minimize loss of life, structures, and natural resources from flooding and erosion.
1. The principal structure is proposed in the Flood Zone AE EI 6.
i' 2. The location of the retaining walls on the property boundary could adversely impact
F' adjacent property owners by interrupting and diverting storm water.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
written determination regarding the consistency of the proposed action.
