NOT CONSENTED TO AN INSPECTION of the premises by the Building Inspec-
tor to determine if the premises comply with all applicable codes and ordin-
ances, other than the Building Zone Code~ and therefore, no such inspection
has been conducted. This Certificate, therefore, does not, and is not intended
to certify that the premises comply with all other applicable codes and regula-
tions.

DOROTHY C. RUSSELL, being duly sworn, deposes and says:
Your deponent is an owner of premises situate at the
corner of Midlothian and Vulcan Avenues, at Fishers Island, Town
of Southold, Suffolk County and State of New York, as more
particularly shown on survey of Chandler, Palmer & King, dated
July 5, 1984, annexed hereto and made a part hereof.
That the premises have always been used as a single
family dwelling. The home and garage have been in existence on
the premises in excess of fifty (50) years to my own personal
knowledge and their use has always been that of a single-family
dwelling.
The condition of the buildings is good as far as I am
concerned, and I respectfully request that the buildings, having
been constructed prior to April 1957, that the Building Inspector
of the Town of Southold issue a pre-existing certificate of
occupancy for the same.
