This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings,property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
B. For existing buildings (prior to April 9, 1957) non-conforming uses,or buildings and"pre-existing"land uses:
1. Accurate survey of property showing all property lines,streets,building and unusual natural or topographic
features.
2. A properly completed application and consent to inspect signed by the applicant. If a Certificate of Occupancy is

What is the best way to meet with you as per the instructions of the
staffers who answer the phone in the building department to move
my application forward, as per your directive that a building permit
is required for the "newer windows and doors"?
I was told to go to the Building Department office from 3:30 to 4:00
p.m. to meet with you. I waited. Finally Mr.
Verity called you to see if you had any intention of coming back to
the office before the 4:00 p.m. closing time.
Not that it is your problem and I realize these things happen but
had to cancel an appointment of my own with a client and reset for
Sat. in order to go over to the Building Department to meet with
you.
As per conversation with the staffers who answer the phone you
don't make appointments. "Just come in at 3:30" which is what I
did.
Please let me know how you are able to meet with me or if
someone else can work on this so we can move forward?
I would like to be in compliance with the law.
On the chicken coop and the shed that was put on the property by
the tenants without my knowledge and needless to say without my
express written permission as per the lease agreement, they are
going to come into the building department Tuesday to pursue
those permits.
On this matter please advise me as to who they need to meet with
and what is the best way to go about this.\
Clearly no one involved wants to run around on a wild goose chase.

I don't think it is unreasonably difficult to have the staff check with
you when you are on the road before they assume you will be back
in the office between 3:30 and 4:00 p.m. particularly if you don't
even know anyone is coming in to the office to meet with you.
So, what do I do so that I can move forward? Who do I talk to,
where do I go and is it beyond reason to makean appointment?
Thank you. I know you were out in the field all day. I was too. I
canceled a meeting to try to be in your office at the instructed time.
I doubt this was even relayed to you. If it was you could have called
and instructed the staff to call me and tell me not to come because
you wouldn't be back.
I have to shuffle appointments and meetings around all day most
days of the week. If I did a no show like this I would loose a client.
For sure.
In this case I have no choice but to try to keep calm and work with
this situation. So again, what do i do, where do I go and who do
talk too.
