The plans and specifications-bearing the stamp and serial number of the New York State Department
of Labor have been examined under the provisions of Sec. 200,4 of the Labor Law with respect to the appli-
cable requirements of the State IBuilding Construction Code and the generally accepted standards there-
under and found to be

occupancy indicated on the application for examination of plans°
This determination is vaIid only on the basis of the statements set forth in the application for exam-
ination of plans bearing the same stamp and number and to the extent that such plans are subject to the
provisions of said law and code and within the jurisdiction of the Industrial Commissioner. Notify the
Building Plans Engineering Unit of any proposed deviations ftora these plans; filing of revised plans may
be required in these instances. These plans .should be submitted for further examination to the local
building official responsible for the appropriate provisions of the State Building Construction Code.
A set of plans should be retained on the premises at all times so that inspectors of this department or
local building officials may consult them in the course of their inspections.
This is not a building permit. To obtain required permit take this certificate, ,with the approved plans,
to the local official.

INSTRUCTIONS
a. This application must be ~omptetely filled in by typewriter or in ink and submitted in duplicate to the Building
Inspector.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets or
areas, and giving a detailed description of layout of propertymust be drawn on the diagram which is part of this application.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such
permit shall be kept on the premises available for inspection throughout the progress of the work.
e. No building shall be occupied or used in whole or in part for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
