As requested by the Southold Town Building Department, this affidavit certifies that Henry
Smith Plumbing Inc. (Plumbers License # 3083MP) did not use any copper piping on the
above referenced garage alteration project at 2220 Skunk Lane in Cutchogue, New York. All
water piping for the outdoor shower and water line was PEX tubing and associated
connector fittings. Therefore no solder was used on the project.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b. Plot plan showing location of lot and of buildings on premises,relationship to adjoining premises or public streets or
areas, and waterways.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application,the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e.No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize, in writing,the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

Dear Chief Building Inspector Verity:
As owner of above referenced property, enclosed you will find a completed building permit
application(signed and notarized) for an unconditioned 2nd story dormer and small addition to the
rear of my garage (accessory building). The dormer(existing garage 2nd floor) will be for storage
purposes(pool appurtenances)as well as for visibility on pool (by separate pool permit submission
—Dunrite Pools). The first floor has a patio, 53 SF changing room(dry) and outdoor shower. The
2nd floor addition is a 115 SF porch off the dormered garage. An 8' wide by 8' deep drywell was
recently installed to capture some runoff from our side parking area and will serve as the backwash
drywell for pool and drain for outdoor shower(exempt from SCDHS requirements). Enclosed you
will find the following documents to support your permit review;

Dear Chief Building Inspector Verity:
As owner of above referenced property, I submitted a building permit application to your
Department on June 4, 2020. After reviewing the plan set that was submitted I realized that the
electric and plumbing improvements were not part of the plan set. Please accept these four (4)
stamped drawings (Sheet A301) to supplement the plans previously submitted. The existing
permitted detached garage already has electrical outlets and lighting (including two GFI exterior
outlets). The limited plumbing is to support the outdoor shower and to have a spigot for
convenience. The two (2) electrical circuits is to have some lighting (consistent with existing
garage layout) and a couple outlets for the porch and storage addition.
Enclosed you will find the following supplemental document to support your permit review;
• 4 - A301 Plans (stamped and sealed).
