PERMIT UNDER SECTION 136 of ARTICLE VI of HIGHWAY LAW
Whereas, Section 136 of 2'*lcle VI of the Highway Law provides: "No street surface or other railroad shall be constructed upon any

This permit shall not be assigned or transferred without the written consent of the Commissioner of Public W~rks.
The work authorized by this pe~it shall be performed under the supervision and to the satisfac~on of the Com-
missioner of Public Works or his representative.
Particular attention is call~ to the necessity of thoroughly compacting the back fill which will ~ required
by the Commissioner of Public Works.
The Com~ssioner of Pubhc Works shM1 be given one week's notice .by said applicant of the date when it in-
tends to begin ~he work authorized by th;s pe~it, and prompt notice of its comple~on.
The said applicant he.by agrees to hold the State, County and Town ha~less on ,account of damages of
kind which may arise during the ,progress of the work authoriz~ by this pe~it or by re.on khereof.
Applicant certifies all persons concerned with actual w~k under this pem~it are duly covered by Workmen's Com-
pensation Insurance ~d the State, County and Town shall be held ha~less on account thereof.
The ~m~ssioner ~ Public Works reserves the right to at any time revoke or annul this permit should the
said applicant fail to comply with the te~s and conditions upon which it is grant~.
The applicant agrees to pay all necessa~ e~nses inadent to supe~wision and inspection by reason of the
granting of such pe~it as certified by the Com~ssioner of Public Works, such pa~ent to be made within ten days
from the rendering of the certified accent.
Work under this pe~it to be commenc~ within thirty days from the date ~ pe~it and continued in an
exp~itious maturer.
The a~licant shall submit a detailed pI~ of st~mcture to ~ built, with a description ~ pr~ea met]lod o~
construction.
It is unaerst~d that should future ch~ges in the alig~ent or grade of the highway require cha~es in the
work covered by this application and pe~it, the a~lic~t shall on ~asongble no~ ~om ~e Co~isaioner of Public
Works make such necessa~F ~anges at his o~ expense ~thin the time so s~ified in notice.

b. Plot plan showing location of lot and of buildings on p remises, relationship to adjoining premises or public streets or

c. The work covered by this application may not' be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the .applicant. Such
permit shall be kept on the premises available for inspection throughout the progress of the work.
e. No building shall be occupied or used in whole or in pa rt for any purpose whatever until a Certificate of Occupancy
shall have been granted by the Building Inspector.
