APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, end.unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installatlons, a certificate of Code Compliance from architect or engineer
responsible for the building.
Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
['pre-existing" land uses:
1.~ Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
A properly completed application and a consent to inspect signed by the applicant.
If a. Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

This letter will supplement a telephone conversation
several weeks ago with Jane Blados of your office.
I had
Our client, Mrs. Ruth Conroy, is selling the house which she
and her late husband owned at 755 Lighthouse Lane in Southold.
As part of its title report, the title company sent us a
copy of Certificate of Occupancy No. Z 1352, dated June 5, 1962.
At the bottom of the CO, there is a handwritten notation stating:
"(no H.D. approval cert)." The initials ~H.D." apparently stand
for Suffolk County Health Department. The attorney for the
purchaser has inquired as to the meaning and effect of that
handwritten notation.
In my telephone conversation with Ms. Blados, she informed
me that she had spoken with you and that you had indicated that
the notation is "not a part of the CO" and "does not have any
bearing" on the CO. I would very much appreciate it if you could
confirm in writing the substance of the preceding sentence and
that the CO is in full force and effect.
