1. Final survey of property with accurate location of all buildings,property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations,a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.
b.Plot plan showing location of lot and of buildings on premises,relationship to adjoining premises or public streets or

c. The work covered by this application may not be commenced before issuance of Building Permit.
d.Upon approval of this application,the Building Inspector will issue a Building Permit to the applicant. Such a permit
shall be kept on the premises available for inspection throughout the work.
e.No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize,in writing,the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

As an addendum to the attached SPUD, the Planning Department has conducted a
review of the proposed modifications pursuant to §280-74 B.(2), and has received a
report from our Wireless Technical Consultant, Cityscape, Inc. (see attached report).
We have the following recommendations for this application:
1. The proposed modifications and up-grades are in compliance with the General
Requirements of§280-70, therefore, we recommend a Building Permit be issued
for this application in accordance with the Structural Analysis Report, dated
September 5, 2018;
2. The Building Permit must state the following:
a. All new vertical feed lines/cables shall be installed inside the monopole
structure;
b. All feed line access ports shall be sealed to prevent access by any birds or
other wildlife;
c. The facility shall remain secured and protected from unauthorized
personnel.

Our office represents New Cingular Wireless PCS, LLC (AT&T) with respect to its
Application to modify its existing public utility wireless telecommunication facility at the
subject premises. The proposed,-mpdifica Lq is part of AT&T's "LTE Upgrade" project
which will allow AT&T to provide better se teci _part

At your request, on behalf of the Town of Southold ("Town"), CityScape Consultants, Inc.
("CityScape") in its capacity as telecommunications consultant for the Town, has considered the
merits of the above referenced application submitted by New Cingular Wireless PCS, LLC
("Applicant") to upgrade its equipment on an existing concealed slick-stick tower. The existing
structure is eighty-nine (89) feet tall. The tower is located at the Orient by the Seas Marina &
Restaurant, 40200 Main Road, Orient,New York, see Figure 1.
Support Structure &Equipment
It is proposed to replace the existing antennas and diplexers at the fifty-seven (57) and
sixty-seven (67) foot levels. This equipment is concealed within the outer walls of the structure
and not visible, see Appendix, Exhibit A.
It is also noted the proposal does not meet the side yard setback requirement of 25 feet,
listed in the Ordinance bulk schedule for business office and industrial districts. However, there
is no change in the existing fence surrounding the compound. Thus, it is not believed any setback
waiver is needed.

Structural Analysis
The Applicant submitted a structural analysis dated, September 5, 2018 and based on
ANSI/EIA/TIA-222-G, Class Il, Category D. The structural analysis indicates that proposed
additional will load the tower to 57:2% while the foundation will be 24.3%, see Appendix,
Exhibit B. It is noted that this is a significant reduction in the previous analysis, which is due to
revisions in the New York wind speed criteria.
Under the Middle-Class Tax Relief and Job Creation Act of 2012 (The Act) any personal
wireless facility collocation, modification or upgrade may qualify for streamlined processing.
Furthermore, under Section 6409(a) it can qualify for administrative approval if the request meets
six criteria, most notably(1) it does not increase the structure height by 10%or 20 feet,whichever
is greater, (2) it does not increase the structure width by 20 feet and (3) it does not require any
excavation outside the existing ground compound or leased area. Section 6409(a) further states
that if an application meets the criteria, the application should be approved and not denied.
CityScape has determined this application complies with The Act and recommends approval,with
the following condition:
1. All of Applicant's feed lines/cables shall be installed inside the monopole structure; and,
2. All ports shall remain sufficiently sealed to prevent access by birds and other wildlife; and,
3. The facility shall remain secured and protected from unauthorized personnel.

SUMMARY& RESULTS
The purpose of this analysis was to verify whether the existing structure is capable of carrying the proposed loading
configuration as specified by AT&T to Shore 2 Shore Wireless. This report was commissioned by Mr. Matt Bartlett of
Shore 2 Shore Wireless.
This analysis has been performed in accordance with the 2016 NYSUC based upon an ultimate 3-second gust wind
speed of 136 mph converted to a nominal 3-second gust wind speed of 105 mph per section 1609.3.1 as required for
use in the TIA-222-G Standard per Exception #5 of Section 1609.1.1. Exposure Category D and Risk Category II were
used in this analysis.
Modifications by GPD (Project#: 2015723.01.48780.02, dated 9/5/2018), have been installed and were considered in
this analysis.
The proposed coax shall be run internal to the monopole in order for the analysis to be valid.

ANALYSIS METHOD
tnxTower (Version 8.0.4.1), a commercially available software program, was used to create a three-dimensional model
of the tower and calculate primary member stresses for various dead, live, wind, and ice load cases. Selected output
from the analysis is included in Appendix B. The following table details the information provided to complete this
structural analysis. This analysis is solely based on this information and is being completed without the benefit of a
detailed site visit.

ASSUMPTIONS
This rigorous structural analysis is based on the theoretical capacity of the members and is not a condition assessment
of the tower. This analysis is from information supplied, and therefore, its results are based on and are as accurate as
that supplied data. GPD has made no independent determination, nor is it required to, of its accuracy. The following
assumptions were made for this structural analysis.
1. The tower shaft sizes and shapes are considered accurate as supplied. The material grade is as per data
supplied and/or as assumed and as stated in the materials section.
2. The antenna configuration is as supplied and/or as modeled in the analysis. It is assumed to be complete and
accurate. All antennas, mounts, coax and waveguides are assumed to be properly installed and supported as
per manufacturer requirements.
3. Some assumptions are made regarding antennas and mount sizes and their projected areas based on best
interpretation of data supplied and of best knowledge of antenna type and industry practice.
4. All mounts, if applicable, are considered adequate to support the loading. No actual analysis of the mount(s) is
performed. This analysis is limited to analyzing the tower only.
5. The soil parameters are as per data supplied or as assumed and stated in the calculations.
6. Foundations are properly designed and constructed to resist the original design loads indicated in the
documents provided.
7. The tower and structures have been properly maintained in accordance with TIA Standards and/or with
manufacturer's specifications.
8. All welds and connections are assumed to develop at least the member capacity unless determined otherwise
and explicitly stated in this report.
9. All prior structural modifications are assumed to be as per data supplied/available and to have been properly
installed.
10. Loading interpreted from photos is accurate to ±5' ACL, antenna size accurate to ±3.3 sf, and coax equal to
the number of existing antennas without reserve.
11. All existing AT&T loading was obtained from previous structural analysis by GPD (Project #:
2018723.01.48780.04,dated 1/29/2018),the provided RFDS and site photos and is assumed to be accurate.
12. The proposed coax shall be run internal to the monopole in order for the analysis to be valid.
If any of these assumptions are not valid or have been made in error, this analysis may be affected, and GPD should be
allowed to review any new information to determine its effect on the structural integrity of the tower.

DISCLAIMER OF WARRANTIES
GPD has not performed a site visit to the tower to verify the member sizes or antenna/coax loading. If the existing
conditions are not as represented on the tower elevation contained in this report, we should be contacted immediately
to evaluate the significance of the discrepancy. This is not a condition assessment of the tower or foundation. This
report does not replace a full tower inspection. The tower and foundations are assumed to have been properly
fabricated, erected, maintained, in good condition, twist free, and plumb.
The engineering services rendered by GPD in connection with this Rigorous Structural Analysis are limited to a
computer analysis of the tower structure and theoretical capacity of its main structural members. No allowance was
made for any damaged, bent, missing, loose, or rusted members (above and below ground). No allowance was made
for loose bolts or cracked welds.
This analysis is limited to the designated maximum wind and seismic conditions per the governing tower standards and
code. Wind forces resulting in tower vibrations near the structure's resonant frequencies were not considered in this
analysis and are outside the scope of this analysis. Lateral loading from any dynamic response was not evaluated under
a time-domain based fatigue analysis.
GPD does not analyze the fabrication of the structure (including welding). It is not possible to have all the very detailed
information needed to perform a thorough analysis of every structural sub-component and connection of an existing
tower. GPD provides a limited scope of service in that we cannot verify the adequacy of every weld, plate connection
detail, etc. The purpose of this report is to assess the capability of adding appurtenances usually accompanied by
transmission lines to the structure.
It is the owner's responsibility to determine the amount of ice accumulation in excess of the code specified amount, if
any,that should be considered in the structural analysis.
The attached sketches are a schematic representation of the analyzed tower. If any material is fabricated from these
sketches, the contractor shall be responsible for field verifying the existing conditions, proper fit, and clearance in the
field. Any mentions of structural modifications are reasonable estimates and should not be used as a precise
construction document. Precise modification drawings are obtainable from GPD, but are beyond the scope of this
report.
Miscellaneous items such as antenna mounts, etc., have not been designed or detailed as a part of our work. We
recommend that material of adequate size and strength be purchased from a reputable tower manufacturer.
Towers are designed to carry gravity, wind, and ice loads. All members, legs, diagonals, struts, and redundant
members provide structural stability to the tower with little redundancy. Absence or removal of a member can trigger
catastrophic failure unless a substitute is provided before any removal. Legs carry axial loads and derive their strength
from shorter unbraced lengths by the presence of redundant members and their connection to the diagonals with bolts
or welds. If the bolts or welds are removed without providing any substitute to the frame, the leg is subjected to a
higher unbraced length that immediately reduces its load carrying capacity. If a diagonal is also removed in addition to
the connection, the unbraced length of the leg is greatly increased, jeopardizing its load carrying capacity. Failure of
one leg can result in a tower collapse because there is no redundancy. Redundant members and diagonals are critical
to the stability of the tower.
GPD makes no warranties, expressed and/or implied, in connection with this report and disclaims any liability arising
from material, fabrication, and erection of this tower. GPD will not be responsible whatsoever for, or on account of,
consequential or incidental damages sustained by any person, firm, or organization as a result of any data or
conclusions contained in this report. The maximum liability of GPD pursuant to this report will be limited to the total
fee received for preparation of this report.
