Dear Mr. Fisher:
Enclosed please find an Application for Certificate of
Occupancy and Consent to Inspection for the above referenced
premises and two original affidavits. I previously submitted a
survey of the premises and a check in the sum of $50.00 for payment
of your fee for the updated Certificate of Occupancy.
The owner of the premises, Marian Cassata and the adjacent
property owner, Pamela Sellars, affir~ that the rear deck, concrete
retaining walls, and concrete block walls were constructed with the
original premises in 1960.
In light of the incomplete record-keeping practices of the
Building Department in 1960, we would respectfully request that the
Certificate of Occupancy dated May 23, 1961 be reissued to include
all structures as shown on the survey dated February 8, 1996
prepared by Joseph A. Ingegno, Land Surveyor (Single family
residence, deck, concrete retaining walls and concrete block
walls). The structures as they presently exist appear in the
records of the Southold Town Assessor. Moreover, in 1973 the
Southold Town Trustees issued permit no. 995 for the dock and ramp
to the present owner Mariano Cassata, at which time they did not
observe any violations due to alterations to the premises.

a. This application must be completely filled in by typewriter or in ink and submitted in duplicate to the Building
Inspector.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving a detailed description of layout of property must be drawn on the diagram which is part of this location.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application the Building Inspector will issue a Building Permit to the applicant. Such
