THIS INDENTURE, made the 5" day of November, in the year 2019, by and between
DOROTHY BERKS. residing at 675 Halyoake Avenue, Orient, New York 11957 ("Grantor"),
and
DOROTHY BERKS, residing at 675 Halyoake Avenue, Orient, New York 11957, as Trustee of
THE DOROTHY BERKS REVOCABLE TRUST ("Grantee").
WITNESSETH, that the Grantor, in consideration of ten dollars and other valuable
consideration paid by the Grantee, does hereby grant and release unto the Grantee and the heirs
or successors and assigns of the Grantee forever,
ALL that certain plot, piece or parcel of land with the buildings and improvements thereon
erected, situate, lying and being at Orient, Town of Southold, County of Suffolk and State of
New York, bounded and described as follows:
BEGINNING at the intersection of the northwesterly line of Platt Road with the southwesterly
line of said Platt Road; from said point of beginning running along said northwesterly line of
Platt Road, South 54 degrees 04 minutes 40 seconds West, 454.82 feet to an iron pipe and land
of Gid's Island Realty Corp., thence along said laud, North 36 degrees 05 minutes 30 seconds
West, 350.0 feet to an iron pipe; thence along land of Gettinger& Trench,North 54 degrees 16
minutes, 00 seconds East, 348.68 feet to land of Boerum; thence along said land of Boerum, the
following two courses and distances: (1) South 37 degrees 05 minutes 50 seconds East, 340.93
feet to an iron pipe; thence (2)North 54 degrees 04 minutes 40 seconds East, 100.0 feet to said
southwesterly line of Platt Road; thence along said southwesterly line, South 37 degrees 05
minutes 50 seconds East, 18.0 feet to the point or place of BEGINNING.
SAID PREMISES being known as and by the street address 605 Halyoake Avenue, Orient,New
York.
SAID PREMISES BEING the same premises conveyed to the party of the first part herein by
Deed dated November 5, 2019 and recorded in the office of the Suffolk County Clerk on
December 23, 2019 in Liber 13041, Page 226.
TOGETHER with all right, title and interest, if any, of the Grantor in and to any streets and
roads abutting the above described premises to the center lines thereof,
TOGETHER with the appurtenances and all the estate and rights of the Grantor in and to said
premises;

TO HAVE AND TO HOLD the premises herein granted unto the Grantees, the heirs or
successors and assigns of the Grantees forever.
AND the Grantor, in compliance with Section 13 of the Lien Law, covenants that the Grantor
will receive the consideration for this conveyance and will hold the right to receive such
consideration as a trust fund to be applied first for the purpose of paying the cost of the
improvement and will apply the same first to the payment of the cost of the improvement before
using any part of the total of the same for any other purpose.
AND the Grantor covenants as follows: that said Grantor is seized of the said premises in fee
simple, and has good right to convey the same; that the Grantee shall quietly enjoy the said
premises; that the said premises are free from encumbrances, except as aforesaid; that the
Grantor will execute or procure any further necessary assurance of the title to said premises; and
that said Grantor will forever warrant the title to said premises.
IN WITNESS WHEREOF, the Grantor has duly executed this deed the day and
year first above written.
