APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Finat survey of property with accurate location of aH buildings, property lines, streets, and unusual
natural or topographm features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3, Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and instal~a-
tions, a cert~hcate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit P~anning Board approval of completed s~te plan requirements where applicable.
For exist;n§ buddings (prior to April i957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buitdings and unusua~ natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or ct.her pertinent informa-
tion required to prepare ~ certificate.

Gentlemen:
I enclose herewith application, in duplicate, for a Cer-
tificate of Occupancy for an existing building erected
prior to 1957, together with a survey, in duplicate, made
by Van Tuyl & Son dated June 28, 1966.
The building shown on the survey was erected, as indicated
on the application, prior to 1957. The department indicated
by phone that a permit for an addition was issued on May 20,
1975 under No. 7943. This application was submitted by Mr.
and Mrs. Smith for the sole purpose of screening an existing
porch and not for an addition. They were advised by your
department that the screening of a porch did not require a
building permit and, therefore, no certificate was issued in
connection with that application.
I also enclose our check for $5.00 for your fee in issuing
the Certificate of Occupancy on a pre-existing building, and
trust that you will issue such a certificate and forward the
same to this office.

a. This application must be completely filled in by typewriter or in ink and suJ~mitted in duplicate to the Building
Inspector.
b. Plot plan showing location of lot and of buildings on premises, relationship to adjoining premises or public streets
or areas, and giving o detailed description of layout of property must be drawn on the diagram which is part of this location.
c. The work covered by this application may not be commenced before issuance of Building Permit.
d. Upon approval of this application, the Building Inspector will issue a Building Permit to the applicant. Such
permit shall be kept on the premises available for inspection throughout the progress of the work.
e. No building shall be occupied or used in whole or in port for any purpose whatever until o Certificate of Occupancy
shall have been granted by the Building Inspector.
