Frank Uellendahl, RA on behalf of DAVID & DIANE NELSON requests a Wetland Permit for the
existing 128'4" x 187' one-story cottage; existing attached 4.6'x5.6' enclosed outdoor shower to be
temporarily removed and replaced in-place; remove and replace existing locust and concrete
footing with as per code minimum 3' deep,,+/-18-12" diameter concrete footings; raise cottage up
+/-2' to code compliant FEMA requirements; construct roof alteration with dormer; replace existing
windows; and replace existing entrance steps with new 3'x8.4' entry platform with 4' wide steps to
ground. Located: 65490 Route 25, Breezy Shores Cottage#24, Greenport. SCTM# 1000-53-5-

The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standar''ds. Based upon the information provided on the LWRP Consistency Assessment Form
submitted to this department, as well as the records available to me, the proposed action is
recommended as CONSISTENT with the LWRP.
The structure is located within Flood Zone AE EI 7 and will be raised to meet Chapter 148 Flood
Damage Protection of the Southold Town Code.
To further Policy 6 it is recommended that the Board assess the location and function of the
sanitary system due to the proximity to surface waters.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
written determination regarding the consistency of the proposed action.

1. All applicants for permits* including Town of Southold agencies, shall complete 's CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistenc eview Law. This
assessment is intended to supplement other information used by a Town Southold agency in
making a determination of consistency. *Except minor ,exempt actions ' eluding Building Permits
and other ministerial permits not located within the Coastal Erosion and Area.
2. Before answering the questions in Section C, the preparer of s form should review the exempt
minor action list, policies and explanations of each policy:c tamed in the Town of Southold Local
Waterfront Revitalization Program. A proposed actio will be evaluated as to its si significant
beneficial and adverse effects upon the coastal area Cwhi' includes all of Southold Town .
3: If any question in Section C on this form is answ red "yes" or "no", then the proposed action will
affect the achievement of the LWRP policy s dards and conditions contained in the consistency
review law. Thus each answer must be ex ained in detail listing both supporting and non-
