Suffolk Environmental Consulting on behalf of ANDREW & LINDA TOGA requests a Wetland
Permit to demolish a 14.25'x23' sunroom located at the southwestern corner of existing dwelling
and reconstruct within the same footprint of existing; construct a 40'x65' (2,036.67sq.ft.) second
story addition; proposed 22.58'x26' attached garage onto landward side of dwelling; a proposed
24'x14' (336sq.ft.) at grade deck on westerly side of dwelling; a proposed 6'x18' (105sq.ft.) covered
porch on easterly side of dwelling; proposed 83sq.ft. of steps and landings off of dwelling;
59.45sq.ft. of wood steps; a proposed 30sq.ft. 2nd story balcony; a proposed 21 sq.ft. cellar entrance;
a 21 sq.ft. outdoor shower; abandon existing cesspool and install a O.W.T.S. septic system
landward of dwelling with an 89 linear foot long retaining wall set 22' landward of the wetland
boundary; and to install gutters to leaders to drywells to contain roof runoff, and in accordance with
Chapter 236 of the Town Code—Stormwater Management. Located: 2425 Mill Creek Drive West,

The proposed action has been reviewed to Chapter 268, Waterfront Consistency Review of the
Town of Southold Town Code and the Local Waterfront Revitalization Program (LWRP) Policy
Standards. Based upon the information provided on the LWRP Consistency Assessment Form
submitted to this department, as well as the records available to me, it is my-recommendation that
the action is CONSISTENT with the Policy Standards and therefore is CONSISTENT with the
LWRP.
Pursuant to Chapter 268, the Board of Trustees shall consider this recommendation in preparing its
written determination regarding the consistency of the proposed action.

1. All applicants for permits* including Town of Southold agencies, shall complete this CCAF for
proposed actions that are subject to the Town of Southold Waterfront Consistency Review Law. This
assessment is intended to supplement other information used by a Town of Southold agency in
making a determination of consistency. *Except minor exempt actions including Building Permits
and other ministerial permits not located within the Coastal Erosion Hazard Area.
2. Before answering the questions in Section C,the preparer of this form should review the exempt
minor action list,policies and explanations of each policy contained in the Town of Southold Local
Waterfront Revitalization Program. A proposed action will be evaluated as toit's significant
beneficial and adverse effects upon the coastal area which includes all o outhold Town .
3. If any question in Section C on this form is answered"yes"or"n ',then the proposed action will
affect the achievement of the LWRP policy standards and con ' ions contained in the consistency

As you may recall, this Firm filed an application on April 24, 2018 with your Board seeking approval to
demolish a 14.25' x 23' foot sunroom located at the southwestern corner of the existing dwelling,
reconstruct within the same footprint and construct a 40'x 65' second story addition (2036.67 s.f.); a 24'
x 14' at grade deck (336 s. f.), a 6' x 18' covered porch (105 s. f.), a 83 s. f. steps + landings, a 59.45 s. f.
wood steps, a 30 s.f. 2"d story balcony, a 21 s. f. cellar entrance and a 21 s. f. shower. Applicant to

Building Department who required issuance of a zoning variance.
Enclosed herewith please find a copy of the Zoning Determination dated December 26, 2018 approving

revised July 17, 2018 referred to in the Zoning Determination for your records.

this proposal is to construct a second story addition with attached decks and porch will not result in adverse

control and a significant upgrade of the septic system. Accordingly,this proiect is consistent with this
policy.

of a silt fence or hay bales encircling the construction activities. Therefore the proposal is consistent with
this policy.
