1. That it is the intention of the undersigned to request a Permit from the Board of
Trustees to:
Construct onto existing one-story,single-family dwelling an 861 sf one-story addition with 5'—8' wide
basement entry stairs and a 57 sf one-story front entry addition accessed by 5.2' x 11' covered steps and
5.2'x 6.9' uncovered steps;remove approximately 1,200 sf portion of existing driveway and install new
pervious gravel driveway located partially in Chapter 275 jurisdiction; and install drainage system of
leaders,gutters,and drywells,all as depicted on the site plan prepared by Nathan Taft Corwin III Land
Surveyor,last dated June 19,2019.
2. That the property which is the subject of Environmental Review is located
adjacent to your property and is described as follows:
335 South Drive,Mattituck
3. That the project which is subject to Environmental Review under Chapters
96,111 and/or 275 of the Town Code is open to public comment on:
