Street Hamlet shown on County tax map as District 1000, Section 018 , Block 06 , Lot 010 does �not)conform to the present Building Zone Code of the Town of Southold for the following reasons: use as an adult rest home was permitted under Board of Appeals, #990, 11/3/66. It is permitted under present zoning, Chapter 100, Art. III, Sec. 100-30 B (4) however, sub -section (a) of this section requires that building setback be 100' from street line & 50' from any line. On the basis of information presented to the Building Inspector's Office, it has been determined that the above nonconforming /T/ Land /X/ Building(s) /_/Use(s) existed on the effective date the present Building Zone Code of the -- Town of Southold, and may be continued pursuant to and subject to the appli- cable provisions of said Code. IT IS FURTHER CERTIFIED that, based upon information presented to the Building Inspector's Office, the occupancy and use for which this Certifi- cate is issued is as follows: an adult rest home. The Certificate is issued to GLADYS ZIMMERMANN ( surviving tenant by (owner, lessee or tenant) the entirety) of the aforesaid building. Gladys Zimmerman being duly sworn deposes and says: That she resides at 30, 300 Main Road, Orient, N. Y. and is the surviving tenant by the entirety of said premises. That deponent has been co-owner of the subject premises since November 19, 1966. Said premises have been operated as an Action of the zoning board of appeals dated November 17, 1966 granting permission to change classification of a non -conforming multiple residence, as premises had been operated since 1949 as an adult residence, is attached hereto and made a part hereof. Also attached is a copy of the December 2 1966 survey; there have been no changes. The porch on the West side of the house is enclosed. The New York State Department of Health periodically inspects the premises as required of adult residences. The last inspection was made in April, 1983, at which time no violations were found. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3.
