Ma'cOc±Ocuok, New York Buildin~ Inspector Housing Code Inspection July 31, 1978 1680 Westview Dr. R-1 Mattituck, New York Tax Roll: Elizabeth Zegzula unoccupied Upon receipt of au application for a Pre-Existing Certificate of Occupancy I made au inspection of the building on the above captioned premises. I picked up the key for this dwelling at the Val Stype Agency on Main Road, Mattituck and began this inspection at approximately 1:25 P.M. This building is a wood frame one story structure constructed on a cement block foundation. The ground floor contains front entry hallway, living room with fireplace, dining room, kitchen, two full bathrooms and three bedrooms. Heat is furnished to all rooms from an oil fired forced warm air furnace located in full cellar of buildings foundation. Cellar is partitioned and one section has been completed for recreational facilities and this area contains a fireplace. A two car garage is located within the cellar of the buildingunder the bedroom end of the dwelling. An in-ground swimming pool constructed under Building Permit # 4930Z is located in the rear yard area. Subsequent Certificate of Occupancy for the swimming pool was issued under the number Z3952, dated August 31, 1970. The following violations of the Housing Code, Chapter 52, Code of the Town of Southold, N.Y. were found: Fireplace - outer hearth in living room - 13 inches in depth minimum of 16 inches required by code. Article IV, Section 52-41 A. Stairway to cellar - door leading to stairway swings into stairwell, no landing provided. Article II, Section 52-27 A. Garage section of cellar - no fire separation between the ceiling of garage and habitable area overhead. Article I, Section 52-15 Reference to New York State Building Construction Code A-402 - 3.6 a. Interior protection - cellar floor wet along the perimeter of the foundation wall (base of wall). Article III, Section 52- 31A. Exterior - Concrete block retaining walls on both sides of garage entry are breaking at motar joints and are giving way to ground pressure. Article III, Section 52-30 A & B. Housing Code Inspection - Zegzula -2- July 31, 1978 Gate of pool enclosure- on the north of premises has sagged and the latch does not line up with the striking plate there- fore the gate does not lock properly. This is a violation of the Zoning Ordinance, Chapter 100, Article III, Section 100- 30C2a. at approximately 1'~15 P.M. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect ar Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), No~-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Dote of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. Fees: 1. Certificate of occupancy $5.00 2.
