JOHN F. deREEDER, being duly sworn, deposes and says:
1. I reside at 240 Wampum Way, Southold, New York;
2. I am a licensed real estate broker in the State of New
York;
3. I am the agent for Howard L. Young, Main Road, Orient,
New York in connection with his application to divide
certain property located at Main Road, Orient, New York,
into building lots;
4. I make this affidavit based upon telephone conversations
I have had with Howard Young between July 19, 1985 and
July 28, 1985:
A. The accessory cottage east of the main house
on the subject property was built prior to
1900;
B. During the year 1955, Howard Young and Ed
Ryder of Orient made modifications to said
accessory cottage so that same could and
would be used as sleeping quarters;
C. The sole exterior modification to the cottage
was the addition of a deck on the north side;
D.
Upon the completion of the modifications in
1955 and prior to April 23, 1957 and
continuously since said date the accessory
cottage has been used

Instructions
A. This application must be filled in typewriter OR ink, and submitted indepill1iiIIIIIIIi to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
C. Fees:
