LOSS CONTROL PROGRAM EXECUTIVE SAFETY COMMITTEE
MEETING MINUTES
Thurs., August 27, 2015
Present: Phillip Beltz, Special Projects Coordinator, Aoifa O'Donnell, National EAP,
John Johnston, Roy Reeve Agency, Jon E. Shearin, Roy Reeve Agency, Josh Albon,

Approval of Minutes
Approved Minutes of April 5, 2015.
Welcome and Introductions
Phillip Beltz welcomed Josh Albon, Workers' Compensation Alliance, Aoifa O'Donnell,
National EAP, John Johnston and Jon Shearin of the Roy Reeve Insurance Agency.
Annual Training
The annual employee training for Workplace Violence Prevention, Bloodborne
Pathogens and Right to Know was completed and a DVD is available for those who
couldn't attend.
Review and Analysis of 2015 Employee Perceptions of Safety
The surveys were not reviewed by the committee due to concerns expressed by several
committee members regarding the process and how information is collected from
employees. Specifically of concern is the ability of employees to make accusations
against others without any appropriate avenue to review complaints and accusations.
Safety surveys also may not list the name of staff making complaint, however Josh
stated that this is a legal requirement whereby staff do not need to identify
themselves. Several members also reported that the current survey is designed only to
assess perceptions of"workplace violence" and not basic"workplace safety." Stephen
agreed to review all of the surveys and vet those that may implicate others while also

thoroughly investigating complaints, possibly with the assistance of the Town Attorney,
Supervisor or Deputy Supervisor.
Site Visit Glatfelter Public Practice-Corrected Actions:
• Police Dept. smoke detector
• Peconic Lane Community Center and Recreation Center smoke detectors
• Highway Dept. and Solid Waste-consolidate flammable liquids
All listed items have been completed.
Update MSDA Global Harmonization Implementation
Josh Albon reminded everyone that the safety data sheets are expected to be updated
by June 2016 and the suppliers should have them available by December 1, 2015.
Site Visit Comp Alliance-to be corrected:-
Jim Bunchuck has budged for footwear for the Transfer Station and will follow-up with
the vendor. He expects to have this accomplished by the end of the year.
Vincent Orlando reported that there is an eye-wash station near his office and the new
highway building will have one as well. He will also make sure the bench grinders in the
Highway Dept. have all their guards.
Josh Albon distributed the training requirements for the employees at the Highway Dept.,
Transfer Station and Dept. of Public Works. He recommended each department head
audit their respective departments for these training requirements.
Housekeeping items at the Transfer Station have all been resolved per Jim Bunchuck.
Remaining 2015 Actions Items:
• Safety alarms systems and surveillance with Town facilities— Lloyd Reisenberg
reported that he is waiting to hear back from Councilman Dinizio with direction
from the Town Board. A commitment from the Board is necessary in order to
move forward. Lloyd will follow-up with Councilman Dinizio and include Jim
Bunchuck in future discussions.
• Justice Court renovations—work in progress.
• Defibrillators—Jeff Standish reported that he ordered five (5) defibrillators as well
as first-aid kits.
• Basement clean-up— Elizabeth Neville reported that there are still mold and
mildew issues in the basement. The Town is looking into a HVAC system,
however, it will be a major capital improvement
• Park and Recreation equipment reporting —John Johnston of the Roy Reeve
Agency required the reports be done weekly by the Recreation Dept. The
department head indicated in the past that he doesn't have time and requested

the reports be completed by the Dept. of Public Works. John Johnston will
provide Phillip with a checklist for the Recreation Director to complete weekly.
• Yak Tracks for HRC and Landfill —will be implemented where feasible.
Lifting —Josh Albon incorporated into the annual training.
Injuries/Supervisor Accident Report
Josh Albon commended the Town on the improvements to the accident investigations.
He recommended that accidents that occur on Mondays be further investigated as there
is a possibility they could have happened over the weekend. Workers' Comp. injuries are
not HIPA protected.
Misc:
Jim Bunchuck announced that the Sharps facility is in place at the Landfill and the
containers are available at the Town Clerk's office.
