IT IS FURTHER CERTIFIED that, based upon information presented to
the Building Inspector's Office, the occupancy and use for which this Certifi-
cate is issued is as follows: Property contains two story; one family, wood
framed dwelling with two one story extensions; an accessory 2 car
garage: sea wall (concrete) asphalt driveway with cement curb and all
situated in the A Residentail Agricultural zone with access to Middle
Road; a town highway.

has been conducted. This Certificate, therefore, does not, and is not intended
to certify that the premises comply with all other applicable codes and regula-
tions.

Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of ell buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply ahd sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

MARGUERITE RUSSELL, residing at 58 Regent Street,
Bergenfield, New Jersey, being duly sworn, deposes and says:
1. I lived at the premises located at (No 9) Wickham
Avenue, Mattittuck, New York shown on the attached survey,
for many years. My grandfather purchased it from Mr. Bayer,
who sold it to Mr. Siebold. The premises is currently
owned by THOMAS YASSO.
2. I lived there for many years and the buildings, as
shown on the attached survey, were constructed many years
prior to 1957. The premises consists of a one and a half
story frame dwelling, frame garage and a one story frame
cottage.
3. To my knowledge, there have been no changes or
alterations made to the buildings that would require a
Building Permit or Certificate of Occupancy, since 1957.
I make this affidavit in support of the THOMAS YASSO'S
application to the Town of Southold for a Certificate of
Pre-Existing Use for the dwelling, garage and cottage.
