APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming u~s, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

JUNE N. HETZER, being duly sworn, deposes and says:
That I am an Executor of the Estate of Grace Co Wylie,
deceased, the present owner of premises located at Main Road, Cutchogue
New York, and designated on the Suffolk County Tax Map as District
1000, Section 109, Block 05, Lot 003, and I am familiar with the con-
dition of same;
That the structures situated thereon have been designed,
arranged and physically occupied as a single family dwelling with two
detached sheds, a barn, and privy;
That at this time the premises is not occupied;
That the condition of the existing structures on the
premises is good;
That I make this affidavit in the knowledge that the
Building Department of the Town of Southold will rely on the accuracy
thereof in issuing a certificate of occupancy for pre-existing use.
