APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitte~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-exi~tin9"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

ROBERT S. DAVIS, being duly sworn, deposes and says:
1. That I am over the age of 18 years and I reside at
no# Sound Avenue, Mattituck, New York and I submit this affidavit
in support of the application of Geraldine B. Davis for a pre-
existing Certificate of Occupancy.
2. I am married to Geraldine B. Davis and have lived at
the subject premise~ identified by Suffolk County Tax Map No.
1000-122-2-14, for over five years. Prior to that time, I lived

in the premises a~jacent/premises on tne east for thirty years and
I am fully familiar with the subject premises.
3. There have been no additions or modfications to the
subject premises that I know of except for the addition of a
deck on the easterly side of the premises for which a building
permit has been obtained from the Town of Southold.
4. I have compared the physical description of the premises
with a survey prepared by Young & Young dated October 31, 1972,
job No. 72-744. There have been no changes in the premises except
for the addition of the aforesaid deck and the premises remain the
same as they have been for over thirty years and prior to April
of 1957.
5. This affidavit is submitted to the Building Department
of the Town of Southold knowing full well that the Building
Department will rely upon the truth of the statements made herein.
