Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1% lead. 5. Commercial building, industrial building, multiple residences and similar buildings and installations, a certificate of Code Compliance from architect or engineer responsible for the building. 6, Submit Planning Board Approval of completed site plan requirements. STATE OF NEW YORK COUNTY OF SUFFOLK ss.: KENNETH WOKOSKY, being duly sworn, deposes and says: 1) That he resides at Church Lane, Box 361, Aquebogue, New York, 11931. 2) That he is the Executor of the Estate of Alice M. wokosky, late of the Town of Southold, County of Suffolk and State of New York, who died on January 7, 1994, having been appointed Executor by the Suffolk County Surrogate's Court on January 28, 1994 under File No. 87 P 94. 3) That he is the son of Alice M. Wokosky and the late Stanley Wokosky, who died on or about March 11, 1957, a resident of the Town of Southold, County of Suffolk and State of New York. 4) That Stanley Wokosky and Alice M. Wokosky were the owners of and resided in a house located on the North side of N.Y.S. Route 25 in Southold, New York, which premises is now known as 57635 Main Road. 5) That as the son of Stanley and Alice M. Wokosky, he was raised in the premises referred to in Paragraph 4, supra, and knows that the northerly areas of the house which are listed on the Town of Southold Property Record Card as 12' x 10' and 14' x 17' foot extensions were in fact built prior to 1957. Indeed, the most northerly such area served as the family kitchen while he was a youth. 6) That he makes this affidavit at the request of the Town of Southold Building Department knowing that it will be relied upon in the issuance of a "pre-existing" Certificate of Occupancy covering the premises referred to in Paragraph 4, supra.
