IT IS FURTHER CERTIFIED that, based Upon information presented to

Certificate is issued is as follows: This 1.43 acre property contains
a one story, one family, wood framed dwelling, with accessory two
car garage; and all situated in A-Residential Agricultural zone,
with access to West Mill Creek Drive.

Instructions
This application must be filled in typewriter OR ink, and submitted ~ ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Su bm it Planning Board approval of comp leted site p lan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

WILLIAM WICKHAM, residing at 1150 Harbor Road, New
Suffolk, New York, being duly sworn, deposes and says:
I am an attorney with the firm WICKHAM, WICKHAM &
BRESSLER, P.C., Main Road, Mattituck, New York. Deponent
makes this affidavit for ROBERT WINTERNITZ, who is the owner
of the premises located at Arshamomaque, Village of
Southold, on the East side of West Mill Creek Drive,
Southold, New York, Suffolk County Tax Map #1000-051.00-
06.00-031.001, as shown on the enclosed survey.
The property was conveyed to Robert & Sybil Winternitz
on October 11, 1952 by William A. & Margaret Richmond. Mr.
& Mrs. Winternitz built the residence in 1953-54 and lived
there until Mrs. Winternitz died in 1987. Mr. Winternitz
continued it as his residence until this summer, when he
moved to Massachusetts. To my knowledge and belief, no
changes or alterations were made to the premises that would
require a Certificate of Occupancy.
I make this affidavit to the Town of Southold for a
Certificate of Pre-Existing Use for the residence and
