That the house on the premises located at 14755 Soundview Avenue,
Southold, New York, Tax Map. No. 1000 - 50 - 3 - 7, was constructed prior
to April 1957, and was owned as a single family residence and has remained
same to date. The house is in fair condition and there is one accessory
building on the premises.
This affidavit is submitted to the Building Department of the Town
of Southold for the purpose of obtaining a pre-existing certificate of
occupancy.
