This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic featu res, 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. R~,. 10-10-78 ~ STATE OF NEW YORK) COUNTY OF SUFFOLK) WILLIAM WICKHAM, residing at 115 Old Harbor Road, New York, being duly sworn, deposes and says: I am the owner of the premises on the south side of Main Road, Cutchogue, shown on the attached survey. My father purchased the property around 1900. At that time, the residence was in the northeast corner of the property. Prior to 1950, I had it moved to its present location. No further changes were made. The garage, which is designated as a barn on the survey, was built prior to 1920. The shed in the southeast corner has been torn down. I make this application to the Building Department of the Town of Southold to obtain a Certificate of pre-existing use for New Suffolk, the house and garage. Sworn to before me this April,~?~87.
