I, Estelle L. Weinrib, being duly sworn, depose and say:
1. I reside at 27 West 86th Street, New York, New York.
2. I am the record owner of property located at 3120
Olejule Lane, Mattituck, New York, County Tax Map No. 1,000,
Section 122, Block 04, Lot 020 (the "Premises").
3. I know, and am familiar with, the Premises, and all
structures thereon.
4. The building on the Premises is a one-story,
one-family dwelling, with a one-car garage, constructed prior
April 23, 1957 (the "Dwelling").

5. The Dwelling is in a habitable condition.
6. On January 20, 1981, Building Permit no. 11019Z was
issued for addition of a deck and bedroom to the Dwelling.
Certificate of Occupancy no. Z10608 was issued July 30, 1981.
7. On July 26, 1982, Building Permit no. 11822Z was
issued to construct an accessory building on the Premises.
Certificate of Occupancy no. z11372 was issued December 16, 1982.
8. I make this Affidvait to induce the Building
Department, Town of Southold, to grant a pre-Certificate of
Occupancy to me in connection with my sale of the Premises to
Anthony L. Macari.

Instructions
A. This application must be filled in typewriter OR ink, and submitted m~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
' tions, a certificate of Code compliance from the A_rchitect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
. topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
C. Fees:
