Upon request of the Southold Town Building Department,
I made inspection of this one story framed dwelling and found
the following violations of Local Law ~1, Housing Code, Town
of Southold. I was admitted to entrance of front porch by
Mrs. Weber and began inspection at approximately 11;20 a.m.
The building consists of an enclosed sun porch, a living-
room and dining room combination, kitchen and bathroom. The
only source of heat is a gas space heater installed and vented
through north wall of living room, which does not heat a~eqnately
aoe~rding to Mrs. Weber.
Entrance to Dorch: No switch to control light at
entry-Section 529b.
Living Room: Being used as prime sleeping quarters,
contains bunk beds to accommadate four persons. (Bldg. does
not have bedrooms.)! Section 209b.
Hot water heater is located in bathroom, water pump contain-
ed in pit-accessable through trap door in kitchen floor-building
on partial crawl space & slab. Building, equipment and grounds
are properly maintained.
Inspection completed at approximately 11:45 a.m.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink., and submitted in duplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
§. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2.. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use

That he resides at 1188 Village Avenue, North Bs/dwin, New York
That he and his wife, Catherine M. Weber, are the ewners of certain
premises situate on the north side of Peconic Bay Blvd. 103.97' west of
Sigsbee Road, Laurel, M~ttituck, Town of Southold, Suffolk County, New York.
That they acquired title to said premises from Rs.Vmand A. He~s
and Helen V. Hess, his wife, on June 2?, 1969. That Rs.vmond A. Hess and Helen
V. Hess, his wife, acquired title to said premises from Ma~m~lllan H. Rudeck
and Frances J. Rudeck, his wife, by deed dated June 23, 1962 and recorded
June 28, 1962 in liber 5188 page 210.
That said premises are improved by a dwelling situate in the rear
of the premises.
That said dwelling has been occupied by thom during the summertime
since the date of purchase sm a s~mer cottage.
That there is no runnfng water service for said premises and
that water is supplied by wells on the premises.
That the dwelling on the premises is at least 30 years old.
That he has been advised by Mr. and Mrs. William Granger, who
reside at Peconic Bay Blvd. and Sigsbee Road, Mattituck, New York, that the
dwelling on the deponents property had been constructed to their knowledge
prior to the date that the Grangers took possession of their home over 30
years ago.
That the dwelling is in good condition.
That the dwelling is lighted by electricity and they have a
Certificate of the New York Board of Fire Underwriters therefor dated
