This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. BOX 728 TOWN HALL SOUTHOLD, N.Y. 11971 Pre -C/O Lnspection May 4°, 1982 557,00 Main Road,.Route 25, Southold R-3, Tax Roll: Raymond Wa-licki & wife County Tax,Map No. Dist..1000-062-03-007 TEL. 765-1802 May 11, 1982 Upon receipt of an application for a Pre -Existing Certif- icate of Occupancy, I made an inspection of the non -conforming building --"tavern with apartment upstairs." This property is located in a B-1 Zone "General Business." District. I began this inspection approximately at 9:30 A=.M. accom- panied by Mrs. Maureen Lenart. This building is a two-story wood frame building, with a full cellar. On the first floor is a tavern consisting of tw.o rooms, two toilet rooms and a kitchen. Also included is a walk-in cooler. The second floor has a four -room apartment and a bathroom. It also has a wood burning stove. There is a full cellar under this building. Located in the cellar is the oil fired, hot water heater. This unit heats the entire building. The gas fired domestic water heater is, also located here. The following violations were.found: gas stove, second floor --no shut off --Ch. 52, 52-52-B 2 stairs --no rail going up --Ch. 52, 52-27-B front door --opens in --Life Safety Code, 5=2.1.1.4.1 --Doors shall swing in the direction of exit travel locks on front door --no panic hardware --Life Safety Code, 5- 2.1.2.1.1--A door shall be so arranged as to be readily opened from the side from which egress is to be made at,all times when.the building served thereby i& occupied no secondary exit or in lieu thereof one or more exit openings for emergency user --Ch. 52, 52-28-C.
