APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Nonconforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topograph lc featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

EDWARD V. FASZEZEWSKI, being duly sworn, deposes and says:
1. That I am the President of VETERANS HEADQUARTERS, INC.,
a corporation organized under the Membership Corporation Law of the
State of New York, having its principal place of business at Boisseau
Avenue, Southold, New York.
2. That I make this affidavit in an effort to secure a
Certificate of Occupancy on a pre-existing dwelling, to wit: Our
building on (no street number) Boisseau Avenue, Southold, New York,
as more fully described on the annexed survey.
3. That the aforesaid building and property was purchased
on the llth day of October, 1948, pursuant to a resolution of the
Board of Directors dated the 7th day of October, 1948.
4. That upon information and belief, the premises have
been used without change and for no other purpose than as a meeting
place for the members and guests of the Corporation since the date
of its purchase.
5. That upon information and belief, the premises have not
been used for any other purpose than permitted by the Zoning
Ordinances.
